Who we are
Your Therapy Place is delivered by YOUR THERAPY PLACE – ALLIED HEALTH Pty Ltd (ACN 651 000 949, ABN 31 651 000 949) and YOUR THERAPY PLACE – POSITIVE BEHAVIOUR SUPPORT Pty Ltd (ACN 679 524 657, ABN 90 679 524 657) (together, “Your Therapy Place”, “we”, “us”).

What we collect
Personal and health information such as: contact details; NDIS number and plan information; referrals and assessments; clinical notes and behaviour support plans; incident/complaints details; billing/payment information; communications and booking history; authorised representative/guardian details.

How we collect
Directly from you or your representative (forms, calls, email, portal), from your support team (e.g., support coordinator, plan manager, other treating providers), and from authorised sources where reasonable and lawful.

Why we use your information

  • Assess eligibility and plan supports

  • Deliver therapeutic and specialist behaviour support (PBS) services

  • Prepare, review and lodge behaviour support plans (where applicable)

  • Arrange bookings, travel and communications

  • Manage billing/claims and administration

  • Quality, safety, incidents and complaints management

  • Meet legal and regulatory requirements (e.g., NDIA, NDIS Commission)

Sharing your information
We may share information:

  • Between our related companies (Allied Health / PBS) where required for service delivery and administration

  • With the NDIA, NDIS Commission, plan managers, support coordinators, other treating providers, and authorised representatives/guardians

  • With state/territory bodies responsible for restrictive-practice authorisations (where relevant)

  • With service partners who support our systems (IT, telephony, records, payroll, finance) under confidentiality/privacy safeguards

  • Where required or authorised by law

Overseas disclosures
Some service partners or group personnel may be located overseas, including Philippines (administration support) and cloud hosting regions outside Australia (US & EU). We take reasonable steps to ensure your information is protected in line with Australian Privacy Principle 8.

Confidentiality
All staff and contractors sign confidentiality agreements and access information on a need-to-know basis with role-based permissions. We will not disclose your information without consent unless required or authorised by law.

Security
We use administrative, physical and technical controls (role-based access, MFA, encryption in transit/at rest, access reviews, backups, secure disposal). If a data breach is likely to cause serious harm, we will follow the Notifiable Data Breaches scheme.

Access, correction and complaints
You may request access to, or correction of, your information at any time. If you have a privacy concern, contact our Privacy Officer. If unresolved, you can contact the Office of the Australian Information Commissioner (OAIC) at oaic.gov.au.

If you choose not to provide information
We may be unable to assess eligibility or provide certain services.
